Payment

Dr. Haddad takes the following insurance:

  • Mass General Brigham Health Plan

  • UnitedHealthcare UHC | UBH

For patients with other health insurance, all visits are paid for out-of-pocket. If needed, Dr. Haddad can provide you with an itemized receipt (or a "Superbill") for the services provided, which you can submit to your insurance company. If you would like to utilize your out-of-network insurance benefits, please confirm your benefits directly with your insurance provider and ensure that these visits are covered.

Dr. Haddad does have a sliding scale to increase access. Please let her know in your New Patient Discovery Call if you would like to access the sliding scale or if the current rate works for you.

BIlling

Fees are due upon completion of the visit. Payment can be made via credit card or direct deposit. Please follow the prompts on the Simple Practice website and enter your billing information.

Medicare and masshealth policies

Dr. Haddad does not bill to, or accept payments from, Medicare (primary or secondary) or Medicaid/MassHealth. Because of this, Dr. Haddad is prohibited by law from accepting patients who have these insurance plans, even if they can self-pay.
